• Copy of Data pages of a valid National Passport.
• 6 Color passport size Photograph (light or white Background only).
• Medical Fitness Certificate issued by a physician recognized by the White List country as authorized to carry out seafarers examinations. ''Panama'' Medical Form is mandatory
• 5 STCW certificates ISSUED BY RECOGNIZED MARITIME TRAINING INSTITUTE.

RATINGS FORMING PART OF A NAVIGATION OR ENGINE WATCH ROOM (DECK AND ENGINE WATCHKEEPING)

• Original Application Form.
• Copy valid National Passport.
• 6 Color passport size Photograph (light or white Background only).
• Medical Fitness Certificate issued by a physician recognized by the White List country as authorized to carry out seafarers examinations. Must use ''PANAMA'' Medical Form.
• Basic STCW & Advance courses, MFA/MC,GMDSS, ROC and ARPA, SSO etc.
• Copy of Valid CDC
• Copy of Valid Passport
• Valid Certificate of Competency (Operational & Management Level)
• Copy of valid Company experience certificates, etc

PANAMA COC DECK & ENGINE SIDE RENEWAL (Operational & Management Level)

• Original Application Form.
• Copy of Data pages of a valid National Passport.
• 6 Color passport size Photograph (light or white Background only).
• Medical Fitness Certificate issued by a physician recognized by the White List country as authorized to carry out seafarers examinations. ''PANAMA'' Medical Form is mandatory.
• 5 Basic STCW & Advance courses, MFA/MC, GMDSS, ROC and ARPA, SSO etc.
